On the day of your appointment, you will be asked the following COVID Safety questions via text:

If you are receiving a service that does NOT require the removal of a mask: 


Question 1: Have you had a new or worsening cough?

Question 2: Have you had a fever?

Question 3: Have you had shortness of breath?

Question 4: Have you recently lost your sense of smell or taste?

Question 5: Have you been in close contact with anyone with these symptoms, or anyone who has been diagnosed with Covid-19 in the past 5 days?

Please communicate with me if you are displaying any of these symptoms, or are able to answer yes to any of these questions. I will happily cancel your appointment for the day without a cancellation penalty in these circumstances. Please wait until you have been symptom free for 72 hours before rescheduling, or please wait at least 5 days since your last exposure to a Covid positive patient.  

If you are receiving a Service that DOES requires the temporary removal of a mask:

Question 1: Have you had a new or worsening cough?

Question 2: Have you had a fever?

Question 3: Have you had shortness of breath?

Question 4: Have you recently lost your sense of smell or taste?

Question 5: Have you been in close contact with anyone with these symptoms, or anyone who has been diagnosed with Covid-19 in the past 5 days?

Question 6: Have you traveled with an airline or cruise ship in the last 5 days, or have you been exposed to someone who has traveled with an airline or cruise ship in the past 5 days?

Question 7: Have you or someone you’ve been exposed to attended an in person daycare, school, college, or living facility in the past 5 days that has experienced a Covid outbreak in the last month?

Question 8: Have you been in a crowded space, such as, but not limited to: inside dining, a concert venue, theatre, gym, or sports arena that was either not requiring masks, COVID vaccination status, or a negative COVID test in the last 5 days?

Please communicate with me if you are displaying any of these symptoms, or are able to answer yes to any of these questions. I will happily cancel your appointment for the day without a cancellation penalty in these circumstances. Please wait until you have been symptom free for 72 hours before rescheduling, or please wait at least 5 days since your last exposure to a Covid positive patient. If you have traveled with an airline or cruise ship, have dined at a restaurant that wasn’t requiring patrons to be vaccinated, ordered food from a restaurant in which staff are no longer required to wear masks, attended or been exposed to someone that attends an in person daycare, school, college, or living facility that has had an outbreak within the last month, or been in a crowded space where COVID precautions were no longer being taken, please reschedule for 5 days after any of these activities. Thank you
